I just got this email from Kia:

"A Kia Visa® Prepaid Card will be mailed later this week to the address above. Please note the card has a term of twelve (12) months but can be replaced for up to five (5) years from the date of issuance if it expires or becomes lost or stolen and unused funds remain. If you have any questions regarding use of the card itself after you receive it, including activation and available balance, please call 855.857.5667 (this number is for card-related questions ONLY ). Any other questions about the reimbursement program should be directed to the Kia Fuel Economy Adjustment Rating Program Center at 855.912.5648.
To receive reimbursement for additional miles your qualified Kia vehicle is driven in the future, simply return to the www.KiaMPGinfo.com website to request an additional reimbursement at the time and mileage interval of your choosing (this process is currently under construction on the website and we anticipate completion by mid-December). You will receive another MPG reimbursement claim ticket to be presented to your preferred Kia dealer for vehicle ownership and updated mileage verification.
Our goal is to make this process as easy and convenient as possible. Meanwhile, please know that we are committed as ever to reducing the fuel consumption and emissions of our vehicles in order to provide greater value and efficiency for you, the owner."
